G&P is an art form.
In fighting a "good" grappler...
1) You must have submission awareness.
2) You must have the proper posture.
3) You must understand how to generate power when your hips are not in play. You do that by lifting your elbows high to deliver the punch or elbow.
4) You must know how to "hip in".
5) Be a guard passing threat.
I think other elements has to do more with personal choice.
I think Backarcher covered it pretty well. The best way to become good at ground and pound is to learn at least fundamental grappling skills, and to especially understand the offensive and defensive possibilites of the guard.
A stand up fighter without grappling skill who attempts to G&P someone with a good guard is most likely to get submitted.

It's a concept of driving your hips forward, when your opponent attempts to either place their feet on your hips when his guard is open and you are standing or you can "hip in" when your opponent's guard is open(ankles uncrossed and feet not on your hips) to pass his guard.
